CRSN Stroke Care
New Initiative Award

   The Champlain Regional Stroke Network invites healthcare teams working with stroke survivors to submit to a Recent Project Initiatives competition intended to recognize sites that are moving towards Stroke Distinction and better stroke care based upon the needs laid out in the most recent CRSN Report Card.

  • A first-place award of $2000 will be given for the top submission based on criteria below, to subsidize members of the team that developed the initiative to attend the 2018 World Stroke Congress, to be held in Montreal on October 17-20. This award offers a contribution of up to a maximum of $1000 per person to attend the WSC, intended to cover registration, accommodation and travel expenses. The award is designed to cover a minimum of 2 people at the winning site, but may also be used to cover multiple registration fees.  
  • A second-place award of $1000 will be given for the runner-up submission based on criteria below, to subsidize a member or members of the team that developed the initiative to attend the 2018 World Stroke Congress. This award offers a contribution of up to a maximum of $1000 per person to attend the WSC, intended to cover registration, accommodation and travel expenses. The award may be used for one person or for multiple registration fees.



The Criteria

  • Assessment of submissions will be based primarily upon the information provided in this form, and may be for either a new initiative or a recent initiative that is ongoing.
  • The awards will be based upon measures undertaken to encourage better stroke care within the 2018 calendar year, and must be completed no later than October 31, 2018.
  • This competition is intended to be between teams who work at sites (includes acute care, rehab, community) that work with stroke survivors. The team element is one of the criteria that this competition will be evaluated on.
  • One member from each of the the winning teams will be given complementary registration to the 2018 Ottawa Stroke Summit (November 2), where they will be asked to present to Stroke Summit attendees a summary of their project (3 slides, 3 minutes) as well as lessons learned from the World Stroke Congress (2 slides, 2 minutes).



The Details

  • All contest submissions must be recieved no later than the deadline of 12 noon EST on June 06, 2018 (New Date!).
  • Funds are intended for out of pocket expenses for individuals only, and are not intended for salary replacement or for institutional access.
  • ***Original receipts*** for your receipt of registration must be supplied within a reasonable time period after being paid.
  • Due to the possible volume of applicants, only successful applicants will be notified, approximately two weeks after the application deadline.
  • Completion of this form does not guarantee that the applicant will receive funds, nor does it guaranteed that, if funded, all requested expenses will be covered. This award may NOT be combined with any other CRSN financial support.
