CRSN Bursary Application


The Champlain Regional Stroke Network will be offering financial awards intended to cover the cost of early bird Registration for the 2018 World Stroke Congress, up to a maximum of $400. This major event is to be held in Montreal on October 17-20. This award is being offered to healthcare professionals of all disciplines working with people post stroke within the CRSN. The awards will be given out at the discretion of the CRSN Education Coordinator and Chair of the Education Committee, and will be based primarily upon the information provided in this form. 

Consideration will be given to applications submitted no later than 12 noon EST on Tuesday May 22.

Note that these funds are intended for out of pocket expenses for individuals only, and are not intended for salary replacement or for institutional access. ***Original receipts*** for your registration must be supplied within a reasonable time period after being paid.

Due to the possible volume of applicants, only successful applicants will be notified, approximately two weeks after the application deadline. Completion of this form does not guarantee that the applicant will receive funds, nor does it guaranteed that, if funded, all requested expenses will be covered. This award may NOT be combined with any other CRSN financial support.
