Cornwall Stroke Prevention Clinic

In December 2011, The Cornwall Community Hospital (CCH) established an out patient Secondary Stroke Prevention Clinic (SPC) under the guidance of the Champlain Stroke Prevention System.  The CCH SPC provides rapid access to diagnostic services; health assessment, diagnosis, treatment and risk factor management to those who have had have a Transient Ischemic Attack (TIA) or a mild non-disabling stroke.

To access the stroke prevention clinic, patients require a referral that can be made by a family physician, a medical specialist or through any of the Stormont, Dundas and Glengarry County Hospital Emergency Departments.

The Stroke Prevention team includes a nurse practitioner and an administrative assistant working in close collaboration with a local Neurologist.  Laboratory and diagnostic investigations are completed on site.

Patients who are referred to the clinic will be contacted directly with an appointment.  To ensure the effectiveness of the clinic, certain tests will be completed prior to the initial clinic visit. At the visit, the patient will meet the nurse practitioner who will perform the initial assessment and discuss risk factors.
