Stroke Awareness Month Award

This award recognizes organizations that are committed to quality stroke care. The recipient will receive all funds raised from the June 2022 Stroke Awareness Trivia event planned with Top Dog Fundraising.

Criteria

To be considered, nominees must demonstrate organizational commitment to stroke care. All submissions must satisfy the following criteria:

• Organization is within the Champlain region boundary.

• Must provide services to support stroke patients in their care and recovery, and/or

• Raise awareness and implement best practices to achieve exceptional stroke patient care, and/or

• Collaborate with other organizations and/or health care professionals and/or patients to support an integrated system of stroke care.

Nominations

Nominations must be submitted to the Champlain Regional Stroke Network using the form below. Regional submissions will be accepted until Friday, April 15. All nominations that fit the criteria will be entered into the pool and one winner will be chosen by the CRSN Steering Committee.

If you are the winner, details of your organization and its role in supporting stroke care will be shared at the June 11, 2022 Stroke Awareness Trivia event.
